The Oregon Department of Human Services (DHS) is making domestic partnership declaration forms available, following Friday’s federal court ruling that allows Oregon's domestic partnership law to go into effect. Forms may be found on the State web site, and at county clerks' offices throughout the state, said Jennifer Woodward, state registrar in the DHS Public Health Division. A 36-year old Grande Ronde woman was killed Friday afternoon when she was unable to stop her car in time to avoid hitting a semi-truck as it pulled onto Highway 99E between Canby and Oregon City, and her vehicle became wedged under the truck’s trailer.   Firefighters worked to extricate the deceased driver and her car, wedged under the trailer of the semi-truck.
