On July 27, 2006 at approximately 3:31 pm., a 1999 Honda passenger car with two occupants was eastbound on Highway 26 near milepost 101 rounding a left curve when it slid broadside into the westbound lane. The passenger side of the Honda was struck by the front of a westbound 1989 Plymouth Voyager van.

The passenger in the Honda, identified as 20 year old Madras resident Gerardo Octavio Robles Arreola, was pronounced deceased at the scene. The Honda's driver, tentatively identified as Rodrigo Reynoso, age 24, from Madras, was critically injured and transported by LifeFlight to St. Charles Medical Center in Bend, Oregon.

The two occupants of the Plymouth van received non-life threatening injuries. The driver Thomas Phinney, 67, from Portland and passenger Patricia Barnes, 65, of Troutdale were transported by ambulance to Mt. View Hospital in Madras.

Oregon State Police troopers from the Bend Area Command office are conducting the investigation, and according to a news release said all persons involved in the accident were wearing safety restraints. Speed and alcohol are being investigated as possible contributing factors in this crash.

Assistance at the scene was provided by Warm Springs Police Department, Warm Springs Fire Department, FBI, and Oregon Department of Transportation, who set up a detour established through KahneeTa.
